- Any relevant letters or reports from your doctor or hospital
- Any X-rays, CT or MRI films and reports
- A list of all medicines you are taking
- Your ACC number, if you have one
- Your insurance details, if applicable

At your consultation
There will be discussion of the surgical and non-surgical options for managing your condition. Surgery is not always required, and you are encouraged to ask questions about any aspect of your treatment.
Radiology (x-ray, CT and MRI) facilities are located on site so as to maximise patient convenience.
Changing or cancelling
If you need to cancel or move your appointment, please telephone the office during business hours on (09) 486 5456.
If your condition is the result of an accident, ACC will cover the cost of your specialist consultation.
